Prior to participating in activities at GSAMA, you or your gaurdian will be required to sign and agree to the following two documents.
GSAMA Code of Conduct
All campers, students, instructors, and families agree to uphold this Code of Conduct. Failure to comply may result in warnings, removal from activities, or dismissal without refund.
- Respect for Others
1.1 Keep hands, feet, and objects to yourself.
1.2 Use polite language and tone. No teasing, name‑calling, or profanity.
1.3 Honor personal space. Always ask before touching another person or their belongings.
1.4 Include everyone. Invite others to join activities and games without exclusion.
- Respect for Counselors & Staff
2.1 Follow instructions the first time they are given.
2.2 Address counselors by name (e.g., “Mr. Lee,” “Ms. Samantha”), not nicknames or titles you invent.
2.3 Ask permission before leaving a group, changing rooms, or using the restroom.
2.4 Report issues—if you’re feeling unsafe or see someone struggling, tell the nearest counselor immediately.
- Respect for Equipment & Facilities
3.1 Handle instruments and gear gently. No dropping, banging, or misuse.
3.2 Use equipment only when supervised. Never touch cords, pedals, mic stands, or amplifiers without a counselor present.
3.3 Keep all spaces clean. Return instruments, sheet music, stools, and cases to their correct spots.
3.4 No food or drinks (other than water in a sealed bottle) near instruments or electronics.
- Safety & Behavior
4.1 Stay within designated camp areas. Do not wander off or enter off‑limits rooms.
4.2 Walk at all times indoors. Running in hallways or stairwells is prohibited.
4.3 Use indoor voices unless instructed otherwise during ensemble or performance drills.
4.4 Emergency drills—listen quietly and follow counselor directions immediately.
- Digital Devices & Recording
5.1 Cell phones and tablets must stay off and put away during instructional time unless a counselor directs otherwise.
5.2 Photography or video recording of others requires prior permission from the camper and a counselor.
- Consequences
- First Infraction: Verbal warning and reminder of the rule.
- Second Infraction: Time‑out from the activity and a brief meeting with a counselor.
- Third Infraction: Parent/guardian notification and possible removal from that day’s activities.
- Severe Violation (e.g., physical aggression): Immediate parent/guardian call and potential dismissal from camp.
GSAMA Liability Waiver and Release
This Waiver of Liability & Release Agreement (“Agreement”) is entered into by and between the undersigned Student/Camper’s Parent/Guardian (“Releasor”) and [Camp Name], its owners, directors, officers, employees, volunteers, agents, and representatives (collectively, “Camp”), in consideration for the Camper’s participation any style of Music Day Camp(“Camp Programs”).
- ASSUMPTION OF RISK
I, on behalf of myself and the Camper named below, acknowledge that participation in musical instruction, ensemble rehearsals, performances, and related activities involves inherent risks (e.g., tripping over cables, instrument handling, loud sounds, moving equipment, physical activity). I freely and voluntarily assume all such risks, whether known or unknown, and accept full responsibility for any injury, loss, or damage that may occur.
- RELEASE & WAIVER
I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE Camp for any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by the Camper, or to any property belonging to the Camper, whether caused by the negligence of Camp or otherwise, during participation in Camp Programs or while on Camp premises.
- INDEMNIFICATION
I agree to INDEMNIFY AND HOLD HARMLESS Camp from any loss, liability, damage, or cost Camp may incur due to the Camper’s participation in Camp Programs, whether caused by negligence of Camp or otherwise.
- MEDICAL AUTHORIZATION
- I certify that the Camper is in good health and physically able to participate.
- In the event of illness or injury, I authorize Camp staff to secure medical care or treatment for the Camper. I agree to be financially responsible for any such care.

- PHOTO & VIDEO RELEASE
I grant Camp permission to photograph and/or record the Camper during Camp Programs and to use such images or recordings in Camp promotional materials, social media, websites, and publications, without compensation or further approval.
- COVENANT OF BEHAVIOR
I have reviewed the Camp Code of Conduct with the Camper and understand that failure to abide by those rules—especially regarding respect for others, equipment, and Camp staff—may result in suspension or dismissal without refund.
- INSURANCE
I understand that Camp does not carry medical or accident insurance for participants. I certify that I have adequate coverage or will cover any costs arising from injury or illness.
- SEVERABILITY
I agree that if any provision of this Agreement is held invalid or unenforceable, the remaining provisions shall remain in full force and effect.
